two completely identical maintenances can be added. probably should be unique by name

DEV branch: ZBX-2827

Added checks when adding or updating maintenances.

Steps to test corrections:
Create
1. Go to Configuration->Maintenance
2. Press "Create maintenance period"
3. Enter maintenance name that already exists and press "Save"
Update
1. Go to Configuration->Maintenance
2. Choose any existing maintenance
3. Edit it's name so it would be identical to one of other existent maintenances and press "Save"
