Consider the following use case:
Two actions are created - one is valid (will be executed with no error), other fails all the time.
In problem section they will be displayed like that:

This made me thinking that both of my actions are in a failed state as there are counter next to status "failures".

I suggest to change this numeric output to two values for failed actions like that: "Failures 1/2" or "Failures 1 of 2" to avoid confusion.
