Steps to reproduce:

1. Create trigger with items from two different hosts
2. Create problem for this trigger
3. Open Monitoring->Problems
4. Click on problem Problem name to open context menu

Result: In context menu you see links to both item's history, but without hostname
Expected: If there are two or more hosts on trigger, all items should be prefixed with hostname.
