Sometimes some templated items dont assigned to hosts

      Centos7, zabbix-4.2.6 from repo.zabbix.com

      Steps to reproduce:

      1. make host
      2. link template mysql_slave to it (XML export file attached)
      3. navigate to host's items and look up to items belongs to mysql_slave template

      Result:

      One templated item:

      Expected:
      Five templated items, just like in attached template:
