Steps to reproduce:

1. Upgrade the Zabbix server RPMs from 4.4.0 to 4.4.5
2. Restart the Zabbix server service and Apache
3. Log into the Zabbix frontend
4. Go to Configuration -> Hosts -> $host -> Templates
5. Select a template in the Link new templates section
6. Observe that there is no longer an add button

The expected behavior should be that the add button remains as it was in 4.4.0

The only way I can see to link a new template is to do it through Mass Update in the Configuration -> Hosts UI
