Steps to reproduce:
The 'Template Module Windows network by Zabbix agent' uses {$IFNAME} as part of the key. This causes issues when you have multiple cards of the same type (eg: Mellanox ConnectX-4 Lx Ethernet Adapter) as it will have duplicate IFNAMES. I have replaced the IFNAME with IFALIAS to resolve this and if it crops up again I will use {$IFNAME}.{$IFALIAS}.
