Steps to reproduce:

1. Install Zabbix 5.0.3 frontend.

Checkbox is not available for the With host verification option during frontend setup process.
Clicking on the name enables or disables this.

Expected:
Checkbox is available, names doesn't acts as a selector for the options. Since in the second example you can just click on space between option name and checkbox and it will chnage value.
