Steps to reproduce:

1. Go to Configuration -> {host} -> Items
2. Find item with type of information "Log", "Text" or "Character"
3. Press "..." button located from the left side of item's name
4. Select "Create trigger" option

Result:

"Create trigger" form is displayed without "Operational data" input field.
Expected:

"Operational data" input field is displayed under "Name" field.
