Proxy address field loses its value once button "Clone" is pressed in Proxy configuration form

      Problem description: The value of field "Proxy address" is lost after pressing the "Clone" button in the Proxy configuration form.

      Steps to reproduce:

      1. Create an Active proxy - make sure You've specified the "Proxy address" field
      2. Open configuration of this proxy
      3. Press "Clone" button

      Result: value of field "Proxy address" has been cleared
      Expected: value of "Proxy address" should still be there
