Steps to reproduce:
1) Navigate to Data collection->Discovery.
2) Press on - Create discovery rule.
3) Add all needed parameters. In "Checks" add type "Zabbix agent" with key like - "long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_long_".
4) check form.

Result:
Radio buttons lined incorrectly.

Expected:
As it was in 6.4

UPD:
There is one more interesting behavior that can be reproduced only in Chrome. Check what happens with modal form when size reduced (Ctrl+mouse wheel down).
