Host address and port values are not preserved for discovered items

      Steps to reproduce:
      1) Create host with Interface and Port.
      2) Add discovery rule with item prototype.
      3) Wait for item discovered.
      4) Navigate to discovered item and open it in edit form.
      5) Press Test button.

      Result:

      Empty host and port fields.

      Expected:
      Host address and Port fields filled with correct values from Host interface.
