Steps to reproduce:

1. configure a netapp host with 2 svm and a volume for each svm that has the same name ( ex: 
   svm_a 
     volume_test
   svm_b
     volume_test
2. wait for discovery to end

Result:

You end up with a single volume and an error on the discovery list stating ( basically ) that it cannot create a duplicate item with the same name

Expected:
  To have both volumes create by the discovery action.
This could be achieved by using the "uuid" identifier instead of the volume name, or at least use some sort of "svm"+"volume name" to identify the volume in an uniq way.
