Steps to reproduce:

1. Open Zabbix server host configuration form,
2. Move to the Macro section, select Inherited and host macro sub-tab,
3. Click on Template link.
4. Accept alert message.
5. Close template form and repeat the scenario.

Result: After each such iteration, the user receives more and more alerts.

Expected: The user should always receive only one alert when switching to another form.
