In the item creation form, the Type of information field is not right-aligned in the same way as the other field names.

Steps to reproduce:

1. Go to Monitoring > Hosts > Items
2. Click Create item

Actual result:
The Type of information field is not right-aligned consistently with other field labels.

Expected result:
The Type of information field should be aligned the same way as the other field names.
