Add an "IT service", linked to a trigger. Note that when you added the service, the trigger box described the trigger in the form "host:description". See the blue arrow in create-service.png. Now, when you edit the service, the trigger box will just say "description".

Also, the "add" button does not seem to work. See the red arrow in create-service.png.
