upgrade to 2.0 does not fix potentially incorrect node_cksum_cksum_1 index

      1.8 created index node_cksum_cksum_1, but later versions created node_cksum_1.

      a) documentation should suggest to drop these indexes (http://www.zabbix.com/documentation/2.0/manual/installation/upgrade)
      b) upgrade patches should recreate correct index

      ZBX-3026 already mentioned this in passing (Chapter #4, named "? ?????????")
