Unable to edit Items

      New zabbix-server-mysql Ubuntu install from your package.

      Attempt to edit (even) the standard Items, or simply just save them without any editing fails with:
      Error in query [INSERT INTO items_applications (applicationid,itemid,itemappid) VALUES ('349','23316','5888')] [Duplicate entry '349-23316' for key 'items_applications_1']
      SQL statement execution has failed "INSERT INTO items_applications (applicationid,itemid,itemappid) VALUES ('349','23316','5888')".

      Please help.
