Template selection form was changed in 2.2.

• before 2.0 - popup list window
• after 2.2 - search form

But currently forward-match is only available for search form. User who is not familiar with Zabbix cannot find correct template or can't understand what templates are on the system.

I suggest to:

• have partial-mach for the form
• revival old-style (popup-list)
