Currently, the template PHP-FPM by Zabbix Agent only supports the ability to monitor a single PHP-FPM pool.  For a host with multiple pools, each pool has a separate status and ping page, so this template is not configurable to be able to track all of that information.

The template would need to be reconfigured to work based on a discovery rule to find all status and ping pages related to each pool.
