Steps to reproduce:

  1. Go to Configuration->Maintenances
  2. Click Create new maintenance period
  3. Go to Periods tab
  4. Click New
  5. Select Period type: Monthly
  6. Compare two positions of Date radio button

Very related ZBXNEXT-2698 asks to rename Date label.
